    • Respiratory Therapist/ Registered Nurse
      Responsible for providing equipment services in accordance with specific physician’s orders, and department policies and procedures. Healthcare team member will perform in all areas of Respiratory care in the home and office setting. Provides services for the setup of new oxygen patients; resupplies oxygen tanks and services and delivers durable medical equipment to patient homes. Helps with inventory system, ordering supplies and cleaning of equipment. Setup CPAP, educate on sleep disorder and monitor sleep apnea patients. Educate and assess all oxygen and trach needs to ensure quality patient care including paperwork and pre-authorizations with insurance companies. Prepares and maintains a record for each patient containing all pertinent information, prescriptions and follow-up documentation. Communicates with physicians regarding patient’s progress. Occasional on-call required.RN with Utah License or CRT/RRT with Utah License. CPR Certifications. Strong written and verbal skills, basic computer knowledge, basic decision and problem solving skills. Self-motivated and willing to work independently. Knowledge of CPAP and BIPAP preferred but not required. Must have current Utah driver’s license with no traffic violations within the last two years.

    • Direct Care Coordinator
      Job DescriptionThe direct care coordinator is responsible for all scheduling for nurses and aides. He/she ensures the floor had adequate staffing at all times and that mandatory staffing ratios are maintained. The DCC also provides back up floor coverage when needed. The DCC assists the DON in gathering data for various committee meetings including psychotropic, infection prevention, incident racking, QA, and antibiotic stewardship. He/she will be responsible for weekly lab audits and assisting in other audits, as requested. The DCC reports directly to the DON and does not have staff who report directly to them other than for all scheduling related functions. The DCC provides weekly productivity reports to the DON and Administrator and identifies areas of concern which need escalated to DON. The Direct Care Coordinator may be asked to participate in the clerical input of new resident admission information and will be responsible for inventory and organization of the clinical supply room.       The Dialysis Registered Nurse coordinates the clinical activities related to direct patient care of chronic renal failure patients. Job duties include:- Administers medications to patients- Performs the delivery of patient care including assessment, planning, implementation, and evaluation of patient outcomes- Monitors and revises patient care plans as needed- Ensures quality patient care and provides compassion to the patient and the patient's family members- Performs other clinical related duties as assignedQualifications:Must have a current Utah RN license and must be CPR certified. Minimum of one year experience in general nursing preferred. ACLS preferred and BLS required. The successful candidate will have excellent nursing and communication skills. In addition, have the ability to function independently as well as a team leader and part of the team. Must have a positive attitude with the ability to interact well with the public. This position requires at least 12 hour shifts starting at 0445 and lasting until all patients leave the clinic. Work may be scheduled in either the Roosevelt or Vernal Dialysis Center.
