      Provide emergency medical services to suddenly ill or injured patients according to state and local guidelines and or protocols. Will be required to take calls and be available when scheduled; responsible for the care of the ambulance and other equipment; and effectively communicate well with patients and staff.       The Home Care Services Nurse will be responsible for the following primary tasks/duties: - Plans, coordinates, and documents care for an assigned caseload of patients. - Utilizes physicians orders and professional skills to meet each patients specific needs in compliance with federal and state regulations, reimbursement guidelines as well as agency policies and procedures. - Participates in performance improvement and continuous quality improvement activities. - Completes discharge paperwork on patients.- Performs any admission or routine patient care needs.- One on-call night per week is required. Time will be spent equally between Home Health and Hospice. Qualifications & Requirements:Must have a current Utah RN license, CPR certified, and capable of lifting 50 pounds.     • Clinical Education Coordinator
      The Clinical Education Coordinator will work directly with the Chief Nursing Officer and department managers to create a formalized training program for all clinical departments of Uintah Basin Healthcare. The successful candidate will be responsible for keeping records of all staff training to include online, offsite, and on site training's. Other duties will include the development of a formalized orientation/mentoring program for the nursing department as well as providing assistance to department managers in the training of core nursing competencies. The Clinical Education Coordinator will also work with our local nursing colleges to assist in developing creative academic/practice partnerships which will benefit both Uintah Basin Healthcare and our academic partners to include eventual status as a adjunct faculty to the nursing schools. The successful candidate must have a current RN license and hold a Bachelors degree in nursing or be enrolled in a Bachelors program by the end of the first year of employment. Candidates must be self-motivated, have a good prioritization skills, and be flexible. Candidates must be able to articulate a conceptual model for developing an education program that encompasses all of the aforementioned items.

      The clinical nurse (RN) is accountable and responsible for the provision and coordinator of competent, evidence-based care, which includes assessment, diagnosis, outcome identification, planning, implementation, and evaluation. The clinical nurse participates in decision-making, teaching, mentoring, and collaboration with the multidisciplinary team to advance the patient plan of care, achieve outcomes and enhance operational efficiency.The position exists to manage clinical outcomes, cost, and customer satisfaction in the daily care assignment and in the operations of the unit. The incumbent assures care for patients and families reflective of the organization values. This position coordinates the nursing and interdisciplinary team for daily patient care assignments.Specific Duties1) Place Peripheral Lines for new patients and provide education on medication regime, and document in Medical Record (Cerner Training preferred). 2) Experience placing Peripherally Inserted Central Catheter’s (PICC lines), or willing to learn and complete the education course for insertion within 6 months. 3) Chemotherapy certified, or willing to get certified within 6 months. 4) Follow the five rights of medication administration with physician’s order. 5) Critical thinking on wound care and the management of wounds. 6) Uses clinical judgment in evaluation activities to meet patient care needs establishing priorities.7) Monitors the safety and effectiveness of the environment and equipment.8) Uses knowledge and practices in accordance with the Standards of Nursing Practice.9) Critical-Thinking Skills: Infusion nurses will need to assess a patient's health, as well as detect changes in symptoms, health or pain, and will need to know when action is necessary.10) Attention to Detail: Infusion nurses administer medicines and work with specific treatments that, if wrong could prove fatal. Attention to detail is crucial.11) Organizational Skills: Infusion nurses will face multiple patients, with differing needs, stages of health and risks. Being organized and knowing how to prioritize will be helpful.12) The ability to provide high level of patient confidentiality.13) Recognize and appreciate cultural diversity at many levels.14) Other duties and tasks assigned.
